In over 8000 consecutive admissions to a tertiary level pediatric center, investigators compared the NAECC definition of ARDS3 to that of the Lung Injury Score (LIS)4 and found good agreement.7 The validity of the NAECC definition also has been tested against lung histopathology findings in children undergoing autopsy following an illness characterized by ARDS.8 In that study of 34 children, 26 had ARDS defined as diffuse alveolar damage, with interstitial and intra-alveolar edema and hyaline membranes. Alternatively, lung injury may arise from a systemic inflammatory illness such as nonpulmonary sepsis (the second most common cause of ARDS), nonthoracic trauma (the third most common cause), hemorrhage, pancreatitis, transfusion, and amniotic fluid embolism. The investigators reported the NAECC definition had a sensitivity of 81% (95% CI, 60 to 92) and a specificity of 71% (95% CI, 70 to 98) and argued that adjustment of the NAECC threshold Pao2/Fio2 ratio to <150 would improve the specificity (86%, CI 42 to 99) of the definition.
